Your insurance coverage will ultimately determine where your treatment takes place, where your laboratory test will be sent for analysis, whether procedures will be performed in the physician's office versus the hospital, which hospital or facility is to be used for surgeries and so forth. You must study and know your insurance plan.

We are a participating Medicare Provider. We accept assignment of all Medicare benefits. As a courtesy we will file your supplementary insurance. You may still be responsible for your Medicare deductible dependent on the requirements of your supplemental policy.

Even though we do not participate with many HMO Insurances, patients that are members of a HMO must bring a referral form from their primary care physician (PCP) authorizing their office visit and/or testing. Patients who fail to do this will not be seen. We will be happy to reschedule your appointment so that you can obtain the necessary authorization. Obtaining this authorization is your responsibility.

Please Note: Women's Pelvic Surgery Center of Orlando / Dr. Kathy Jones, participates in a variety of networks, each plan different from the other. The staff does try to keep abreast of the different plans coverages, however, due to constant changes with insurances, it is ultimately your responsibility to know what your insurance plans covers. If the insurance company does not pay the full balance or denies the claim for non covered services, any unpaid balance is due within thirty (30) days of the date of service.

Please call us with any questions about insurance plans accepted or other billing concerns. Below is a current list of the plans we accept (please note this may change without notice). Insurance coverage and benefit questions should be directed to your insurance company. It is your responsibility to notify us of any changes in your insurance plan, ID or policy numbers, group numbers, etc. The insurance companies requests that claims be processed within a certain time limit. In the event you have not updated your information in our files, and your claim is denied for not being filed in a timely manner, the balance will be your responsibility.
